ラジオボタンにhttp://www.somacon.com/p143.php javascript を使用して、選択されているラジオボタンに応じて送信 onclick location.href を変更しています。しかし、ボタンが適切に機能していないため、構文に問題がある可能性があると思います（ラジオボタンから適切に値を取得しているとは思いません）。ありがとうございます！
