Carrierwave アップローダーでデフォルトの画像を設定する際に問題があります。URL の前に奇妙なクラスを追加しているようですが、画像をレンダリングしていないようです。以下の私のコードを参照してください。

# version :thumb do
# process :scale => [50, 50]
# end
# Add a white list of extensions which are allowed to be uploaded.
# For images you might use something like this:
def extension_white_list
%w(jpg jpeg gif png)
end
# Override the filename of the uploaded files:
# Avoid using model.id or version_name here, see uploader/store.rb for details.
def filename
if original_filename
@name ||= Digest::MD5.hexdigest(File.dirname(current_path))
"#{@name}.#{file.extension}"
end
end
end

これが機能した編集済みのコードです。Assets/images/fallback ではなく public/fallback に画像を配置する必要がありました。
