Web アプリをホット リスタートするときに、休止状態の SessionFactory を閉じるのを忘れているようです。これにより、c3p0 のヘルパー スレッドがデッド インスタンス (Tomcat では壊れた ClassLoader を意味する) から無駄に実行され、問題が発生します。

c3p0 DataSource は、使用されなくなったらすぐに close() して、プールされたすべての接続とヘルパー スレッドを解放する必要があります。Web アプリの場合、これは多くの場合、ServletContextListener を介して行う必要があります。プールが休止状態によって管理されている場合は、SessionFactory をすぐに閉じることでうまくいくはずです。
