register メソッドで、'$use' パラメータを 'encoded' ではなく 'literal' にしてみてください。

nuSoap の代わりにネイティブ PHP SoapClient を使用してください。過去の遺物です。

サービス クライアント (Silverlight?) は、サービス呼び出しの結果がtext/xmlUTF-8 エンコーディングであると想定しているようですが、PHP はそれを として返しますtext/html。コマンドtext/htmlで別のコンテンツ タイプを指定しない限り、PHP のデフォルトのコンテンツ タイプです。header

そのため、PHP ファイル/サービスの先頭に以下を追加してみてください。

header('Content-Type: text/xml');

また、テキスト エンコーディングが UTF-8 であることを確認することもできます。
