mapView.visibleMapRect をオーバーレイの実装に渡し、それを boundingMapRect として返します。今のところは問題ありません。マップの四角形全体に線を描画しようとしています。

CGContextAddLineToPoint への呼び出しで座標が間違っています。あなたが持っている：

CGContextMoveToPoint(context, theRect.origin.x, theRect.origin.y);
CGContextAddLineToPoint(context, theRect.size.width, theRect.size.width);

ただし、次のようにする必要があります。
